Answer:
The main cause often comes down to human factors. Here’s a breakdown of the key reasons:

1. Over-Reliance on Automation:

- Our dependency on automated systems has led to fewer physical inspections. Many issues go unnoticed when hands-on checks are reduced.

2. “Tick-Box” Safety Checks:

- Safety checks have become a formality for many, just a “tick in the box,” with limited focus on actual inspection and verification.

3. Ignoring Competency and Attitude:

- Qualifications, skills, and safety attitudes of workers are often overlooked. Regular review and evaluation of personnel are crucial.

4. Lack of Detailed System Health Reports:

- As a consultant, I emphasize the need for in-depth system health reports, documenting specifics like pipeline material, lengths, bends, isolation systems, and testing status. This level of detail is often missing, with more focus on digital representation than practical assessments.

5. Fragmentation of Safety Roles:

- Originally, EHS&F (Environment, Health, Safety & Fire) was a unified responsibility. Division into separate branches for profit has reduced accountability and left gaps in effective risk management.

6. Limited Forensic and Analysis Support:

- In India, forensic support for fire analysis within fire departments is lacking. Proper investigation and analysis are delayed or neglected, limiting corrective actions.

The Bigger Issue:
- These issues have been raised at forums, but cooperation between government bodies, industries, and safety professionals is still lacking. As a consultant and trainer, I continue to advocate for unified efforts, but without wider support, these incidents will continue to rise.

- Remember: Safety is not just about compliance. It’s about valuing and understanding its true importance. If we continue to overlook this, preventable incidents will keep happening, and we’ll always be searching for reasons why.
